Service Coordinator helps resident locate services
|
Several Wesley communities are fortunate to have Service
Coordinators who are responsible for assuring that elderly
and disabled residents are linked to needed supportive services
to enhance their quality of life, continue to live independently,
and avoid premature or unnecessary institutionalization.

These Service Coordinators plan and coordinate social services
and provide Resident Services training to Wesley's elderly
and disabled residents. The coordinators meet with residents
both in groups and individually and may refer them to management,
nutrition, family living, leisure activities and healthcare
services.
They work with residents and Wesley management to help prevent
evictions due to lease violations. They establish volunteer
support so that additional services reach more residents.
They help to educate and make residents aware of service availability,
application procedures and how to access them. They bring
in new providers and maintain current ones.

The Service Coordinators encourage resident involvement in
decision making for themselves, formation of resident councils
and in coordination of services and social events. Residents
who use the services of Service Coordinators have a better
quality of life in their individual home and in their Wesley
community. |
